The Ministry of Community and Social Services established the Community Services Coordination Network (CSCN) to support the coordination of community services. Within the developmental service sector, one of the responsibilities of CSCN is to provide a single point of access for individuals who are seeking accommodation supports. All requests for accommodation supports must be made through CSCN. This single point of access takes into account individual/family choice of service agency they prefer to receive service from. Families can call us directly or they can ask the agency they are working with to contact us on their behalf.

For adult individuals with physical disabilities seeking support:
Individuals with physical disabilities seeking accommodation supports are invited to apply directly to Participation House Support Services London and Area. Individuals and/or their families can call us directly at 519-660-6635 to discuss individual needs.
Project Hope (Help with Opportunities for Participation and Enrichment) is a full-day, flexible program option for severely developmentally challenged and physically challenged young adults who are no longer eligible for educational support, who live at home with their families and who are totally dependent on others for their care, safety and well-being. To apply for Project Hope support services, download the application and forward the completed form to Participation House Support Services.
